About the Role
We're looking for Community Car Coordinators and Community Care Coordinators with lived experience for our Suicide Prevention After Care service.
The Suicide Prevention - After Care Coordinated Support service provides one-on-one care coordination for people of all ages who have attempted suicide and have been discharged from an emergency department, acute setting or following admission to hospital who can be supported in community and primary care settings within the CESPHN region.
Ensure seamless access and provision of multiple services to consumers with complex needs. The Partners in Recovery program aims to support consumers with a severe and persistent mental illness, who also have many needs, and their carers and families to access services and supports from multiple sectors.
As a Direct Support Worker with Me Well your mission will be to empower and support people in the community living with mental illness and to assist these customers in getting the most out of life.
