As the Peer Health Coaching (PHC) comes to the end of the pilot period, we celebrate the achievements of the project for consumers wishing to work on a specific physical health goals.
Peer Health Coaching is a short-term programme for consumers wishing to work on a specific physical health goal. In partnership with SANE Australia, Neami National delivered a sucessful 3-year pilot programme of PHC, addressing the poor physical health outcomes of people living with a mental illness.
Peer Health Coaching surpassed the initial targets with 186 consumers participating over the short pilot programme. Our qualitative and quantitative evaluation shows that consumers participated in PHC achieved their health outcomes or felt confident to pursue them independently afterwards.
We were thrilled to have the programme recognised at the 2015 TheMHS conference with Peer Health Coaching winning the conference award for Physical Health and/or Primary Care.
Watch our Health Literacy video featuring the a consumer and Peer Health Coach discussing their experiences and what the programme meant to them.
